Two Scaffold Collapses in One Day, Both During Setup and Teardown
A scaffold is a structure that spends part of its life half built. Everyone plans for the finished version: the guardrails, the planking, the posted load rating, the green tag hanging at the access point. Far fewer crews plan for the two hours it does not have any of that yet, or the two hours after somebody starts pulling it apart.
Two separate incidents on Monday, September 7, 2026, both happened in exactly that window.
What happened on September 7
At about 11:23 a.m. in Marin County, California, scaffolding and display screens collapsed during event setup at Summit Skywalker Ranch on Lucas Valley Road. Five men were injured. Three were taken to hospitals, one transported code three with a reported head injury, and one had been trapped beneath LCD screens before crews freed him. Marin County Fire, San Rafael Fire and Novato Fire responded. The California Division of Occupational Safety and Health is investigating and no cause has been released. No employer has been publicly named.
That same afternoon in Washington, D.C., a worker was injured while removing scaffolding from the White House North Portico. The scaffolding and drapes had covered the portico for roughly two months while crews stripped paint from the stone columns and worked on the entrance. D.C. Fire and EMS performed a high angle rescue, reaching the worker by ladder and bringing him down by stretcher and basket, and described the injury as non life threatening. Reporting did not establish whether he fell, had a medical event, or was hurt some other way.
Why erection and dismantling is the exposure
The reason is structural rather than cultural. A completed scaffold is a designed system: the base is plumb and on adequate footing, the frames are braced, the platforms are fully planked, the guardrail system is continuous, and a competent person has inspected it. Every one of those protections arrives at some point during the build and departs at some point during the teardown. In between, workers are on a partially assembled structure whose stability depends on components not yet installed.
Dismantling is the harder half. On the way up, the crew is adding capacity to a structure they are building to a plan. On the way down, they are removing capacity from a structure that other trades have been modifying for weeks. Planks get moved. Braces get pulled to let a duct through. Ties get cut for a facade panel and never get replaced. The scaffold that comes down is frequently not the scaffold that went up, and the dismantling crew often has no record of what changed.

What the standard actually requires
Subpart L of 29 CFR 1926 covers scaffolds in construction, and two provisions land directly on this window.
29 CFR 1926.451(f)(7) states that scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Note the two words people drop: supervision and direction. A competent person who is on the property but not directing the work is not what the standard describes. Note also that altered is in the list, which means the trade that pulls a brace to run conduit has just triggered the same requirement as the erection crew.
29 CFR 1926.454(b) requires the employer to have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. Worth noting the contrast inside the same section: 1926.454(a), the training for employees who simply work on scaffolds, is delivered by a qualified person. The erection and dismantling training in (b) specifically names a competent person, which is a higher bar because a competent person must also have authority to correct hazards.
Fall protection during this work has its own provision. 1926.451(g)(2) requires a competent person to determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds. That is a documented determination, not an assumption, and "we could not tie off" is only an answer if somebody actually evaluated it.

On site checklist for erection and dismantling
Name the competent person out loud before work starts, and make sure the crew can repeat the name back. A competent person nobody can identify is not supervising anything.
Confirm that person is qualified specifically in erection, moving, dismantling or alteration, not just qualified to inspect a completed scaffold. Those are different competencies and the standard distinguishes them.
Walk the structure before dismantling and record what changed since it went up. Missing planks, cut ties, removed braces, added loads. Assume it is not the scaffold on the original drawing.
Make the fall protection feasibility determination explicitly and write it down, including what was evaluated and what was chosen.
Control the area below. Both of the September 7 incidents involved people in the collapse zone, and dropped components during teardown are the predictable hazard.
Treat any alteration by another trade as a stop and re inspect trigger. If an electrician pulls a brace on Tuesday, the scaffold is not tagged safe again until the competent person says so.
Verify the erection and dismantling training records for the actual crew on site today, not for the company in general.
